Course Summary

Despite all your best efforts, accidents and incidents still occasionally occur. These unplanned and undesired events will adversely affect your operations, may include death and injury, damage from spills and fires, resulting in loss of confidence and reputation.   Critically all incidents should therefore be thoroughly investigated demonstrating a proven company-wide process of efficiency and effectiveness linked to the Safety Management System (SMS). The more serious the incident the more extensive the investigation.

We will demonstrate how Accident Investigation & Reporting now mirrors the growing sophistication of modern SMSs.  Indeed, as safety management becomes even more advanced, then the need for a more sophisticated Accident Investigation becomes obvious. By retaining Accident Investigation & Reporting in-house it will be essential for your organisation to have a pre-identified, fully trained, confident, and professional team able to conduct a thorough Accident Investigation as soon as practicable after the event.  Attendance to this course will provide you with a clear understanding of how and why things went wrong, to identify any deficiencies in your Risk Control and Safety Management Systems and ensure it never happens again.

  • Report writing techniques which will ‘persuade’ and ‘empower’ actions
  • Identifying deficiencies in your Risk Control & Safety Management System
  • Fully understanding the six steps of Accident Investigations and Reporting
  • How to establish rapport with witnesses and obtain factual statements
  • Incident & Accident Analysis Overview, exploring Accident Theory
  • Unsafe Acts V. Unsafe Conditions: Avoiding the “Blame Game”
  • Scene Protection and Examination - Who should be involved at this stage
  • Listen and Learn, and Learn to Look and Listen More
  • What questions to ask to confirm “Root Cause”
Certificates
  • Upon successful completion of the classroom-based training course, GLOMACS Certificate will be awarded to the delegates. Continuing Professional Education credits (CPE): In accordance with the standards of the National Registry of CPE Sponsors, one CPE credit is granted per 50 minutes of attendance
  • Upon successful completion of the online training course, a GLOMACS Certificate will be awarded to all delegates. Guided Learning Hours – In accordance with ISO 9001:2015–certified quality management standards, one Guided Learning Hour is awarded for every 60 minutes of attendance.
